INITIAL INVESTMENTS

ACCOUNT OPENING REQUIREMENTS

For individuals 

1.    Please accomplish the following forms:

•    Account Opening Form - Individual (2 copies)

•    Investment Application Form (1 copy)

•    Investor Risk Profiling Questionnaire (1 copy)

•    Signature Card (1 copy)


2.    Please submit a photocopy of 2 valid IDs (at least 1 government-issued ID).  Valid IDs are those which contain a photo of yourself, your complete name and your visible signature and have not expired.

3.   The above-mentioned forms may be downloaded.  Once downloaded, please print out the required number of copies indicated above and sign each copy.

4.   Please download and thoroughly read the prospectus of the selected mutual fund.


For institutions

1.    Please accomplish the following forms:

•    Account Opening Form - Institutions (2 copies)

•    Investment Application Form (2 copies)

•    Signature Card (2 copies)

2.    Please submit the following documents:

•    Certified True Copy of:

o    SEC Registration

o    Articles of Incorporation

o    By-Laws

•    Board Resolution

o    States that Corporation is authorized to invest in Philequity

o    Lists authorized signatories

•    Two (2) valid IDs of all authorized signatories. Valid IDs are those which contain a photo of yourself, your complete name and your signature and have not expired.

3.    The above-mentioned forms may be downloaded.  Once downloaded, please print out the required number of copies indicated above and sign each copy.

4.     Please download and thoroughly read the prospectus of the selected mutual fund.


ACCEPTABLE MODES OF INVESTMENT

The following are the acceptable modes of investment:

 1.    Check made payable to the fund of your choice. 

 2.    Direct deposit/ remittance to the fund’s settlement account  Should you opt for this mode of investment, you will need to submit the original, validated deposit slip or remittance slip.  Please note that we do not accept cash at our office.   

BOOKING CUT-OFF TIME

Booking cut-off is at 12 noon and the NAVps which comes out in the afternoon applies.  All investments received or submitted after the 12nn cut off time will be automatically booked on the next banking day and the NAVps that will be applied is the NAVps that will come out on the same booking date.

Please note that booking of regional check payments will be done after the clearing of the check (7 banking days).

SUBMISSION OF REQUIREMENTS

1.     Visit Philequity Headoffice at Unit 2004A, 20/F, East Tower, Philippine Stock Exchange Center, Exchange Road, Ortigas Center, Pasig City

2.     Visit any eBIZ Western Union Branches nationwide (show directory)

3.     If you are based outside Manila or the Philippines, you may send the documents via courier.


CONFIRMATION OF INVESTMENTS

You will be receiving from Philequity Management, Inc. a Confirmation Notice, which should reach you within 10 banking days,  as confirmation and as summary of pertinent details of your transactions with Philequity.

Should you wish to receive your Confirmation Notices, along with your Statements of Account, more speedily, we highly encourage you to enroll in our Electronic Statement and Electronic Confirmation (E-Statement & E-Confirmation) Facility. This facility allows you to receive your Confirmation Notices, along with your Statements of Account, via e-mail, immediately the next banking day. The documents shall be attached as password protected PDF files for added security. You may download the Enrollment Form and Terms & Conditions from the Online Forms section of this website, both of which should be signed in order to be successfully enrolled in the facility.


ADDITIONAL INVESTMENTS

REQUIREMENTS

The following are the requirements to make additional investments:

1.    Investment Application Form (1 copy)

2.    Funds

o    Check made payable to the fund of your choice.

o    Direct deposit/ remittance to the fund’s settlement account  Should you opt for this mode of investment, you will need to submit the original, validated deposit slip or remittance slip.  Please note that we do not accept cash at our office.  You will need to deposit this to the fund’s settlement account.

o    For those with BDO or Unionbank of the Philippines' internet banking facility, fund transfer to the fund’s settlement account and print out the cyber receipt.

REDEMPTIONS

To sell your shares in your chosen mutual fund, you need to submit the following:

1.   Redemption and Stock Assignment Form (2 copies)

2.   Original share certificates with the investor's signature at the back of each certificate (For certificated investments only.  To find out how to convert your shares to scripless form, click here)

BOOKING CUT-OFF TIME

Booking cut-off is at 12 noon and the NAVps which comes out in the afternoon applies.  All redemption requests made after this time will be processed on the following banking day and the NAVps the next afternoon will be applied.

REDEMPTION PROCESSING TIME

Maximum of seven (7) banking days

FORMS OF PAYMENT
You may opt to:

1.    Receive a check made payable to the names of the shareholders

2.    Instruct Philequity Management, Inc. to deposit your proceeds directly to your bank account (Bank Account Name and Philequity Account Name must be the same)


REQUIREMENTS FOR LOST CERTIFICATE/S:

a)     Notarized Affidavit of Loss with undertaking, executed by registered owner, specifying the name of stock, name of registered owner, number of shares, cert. no. and circumstances of the loss.

b)     Affidavit of Publication executed by the publisher of a newspaper of general circulation in the principal office of Issuer Corporation stating that the required Notice of Loss had been published once a week for three consecutive weeks

c)     One-year surety bond to be posted in favor of BDO in the amount twice the market value of the total shares lost, otherwise, issuance shall be made one (1) year after the last date of publication

d)     Transfer fee of Php100.00/certificate and cancellation fee of Php20.00/certificate.
