Find out the procedures for:



ACCOUNT OPENING

  1. Download a copy of our prospectus
  2. Accomplish and submit the following account opening documents based on your account type:

    For Individual/Joint

    1. Account Opening Form and Investment Application Form
    2. One (1) Valid ID with Signature of each investor
    3. Proof of latest billing statement that matches your present/permanent address on the Account Opening Form

    Additional requirements for In-Trust-For (ITF):

    1. In Trust For Account Form
    2. PSA Birth Certificate of beneficiary

    Important:

    For Joint account please indicate whether "OR" or "AND" taking note that in an "AND" account, both shareholders need to sign on every instruction whereas for an "OR" account, only one (1) investor is required to sign.

    All account holders should indicate their Tax Identification Number (TIN) in the account opening forms.

    The shareholder will receive a Confirmation Notice for the investment/redemption made. If enrolled in the email facility, this will be sent within 24 hours from booking date otherwise, it will be coursed through mail which will take more than 7 days. Immediately inform our office for non-receipt of the Confirmation Notice.


    For Institution

    For Sole Proprietorship

    1. Account Opening Form and Investment Application Form
    2. One (1) Valid ID with signature
    3. Certified True Copy of DTI Certificate of Registration
    4. Certified True Copy of Business Permit

    For Partnership

    1. Account Opening Form and Investment Application Form
    2. One (1) Valid ID with signature of each partner
    3. Certified True Copy of SEC Certificate of Registration
    4. Certified True Copy of Partners Agreement
    5. Certified True Copy of Partner's Resolution authorizing investment in Philequity and authorized signatories
    6. Latest General Information Sheet

    For Corporation

    1. Account Opening Form and Investment Application Form
    2. One (1) Valid ID with signature of each authorized signatory duly authenticated by Corporate Secretary
    3. Certified True Copy of SEC Certificate of Registration
    4. Certified True Copy of Articles of Incorporation and By Laws
    5. Certified True Copy of Secretary's Certificate / Board Resolution authorizing investment in Philequity and authorized signatories
    6. Latest General Information Sheet
    7. Valid ID of Corporate Secretary with three (3) signature on the side

    Cooperative

    1. Account Opening Form and Investment Application Form
    2. One (1) Valid ID with signature of each authorized signatory duly authenticated
    3. Certified True Copy of Certificate of Registration from the Cooperative Development Authority
    4. Certified True Copy of Articles of Association or Constitution and By Laws
    5. Certified True Copy of Secretary's Certificate / Board Resolution authorizing investment in Philequity and authorized signatories

  3. Invest through the following mode of investment:
    1. Check — indicate name of the fund (i.e. "Philequity Fund, Inc.") in the "Payable to" portion
    2. Deposit directly to any of our accredited bank accounts.

    Important:

    Important: For investments in US dollar or to Philequity Dollar Income Fund we recommend transacting your deposit with Unionbank since their US Dollar cash deposit is not subject to clearing.


  4. Submit fully accomplished account opening documents together with your proof of investment (i.e. cheque, deposit slip or remittance slip) personally to our office or to any of our designated eBiz branches.

     

    Important:
    1. Cut-off time is 12:00NN. Documents received within this time will be processed with the NAVPS of the same day. Documents received past cut-off will be processed the following business day.

ADDITIONAL INVESTMENTS

  1. Accomplish Investment Application form (IAF)
  2. Invest through the following mode of investment:
    1. Check — indicate name of the fund (i.e. "Philequity Fund, Inc.") in the "Payable to" portion
    2. Deposit directly to any of our accredited bank accounts.

  3. Submit fully accomplished Investment Application Form (IAF) together with your proof of investment (i.e. check, deposit slip or remittance slip) through the following:
    1. If enrolled to our Electronic Mail Instructions – email scanned copy to invest@philequity.net
    2. If not enrolled in our Electronic Mail Instructions - submit original documents to our office or to any of our designated eBiz branches.

    Important:
    1. Cut-off time is 12:00NN. Documents received within this time will be processed with the NAVPS of the same day. Documents received past cut-off will be processed the following business day.

REDEMPTIONS

  1. Accomplish Redemption Order Form and Settlement Account Form
  2. Provide a copy of one (1) Valid ID with signature
  3. Submit all documents and original copies of stock certificate (if certificated) to either one of the following methods:
    1. If enrolled to our Electronic Mail Instructions – email scanned copy to invest@philequity.net
    2. If not enrolled in our Electronic Mail Instructions - submit original documents to our office.

    Important:
    1. Cut-off time is 12:00NN. Documents received within this time will be processed with the NAVPS of the same day. Documents received past cut-off will be processed the following business day.

    2. Redemption processing is up to a maximum of seven (7) banking days.

    3. Forms of payment – you may opt to receive your redemption proceeds as:
      1. Cheque payable to the name of the shareholder
      2. Direct deposit to the shareholder’s bank account

        Note: Redemption proceeds shall only be paid to the             account holder/s.

    4. The shareholder will receive a Confirmation Notice for the investment made. If enrolled in the email facility, this will be sent within 24 hours from booking date otherwise, it will be coursed through mail which will take more than 7 days. Immediately inform our office for non-receipt of the Confirmation Notice.

FUND SWITCH

  1. Accomplish a Fund Switch Form
  2. Submit fully accomplished form to either one of the following methods:
    1. If enrolled to our Electronic Mail Instructions – email scanned copy to invest@philequity.net
    2. If not enrolled in our Electronic Mail Instructions - submit original documents to our office.

AUTOMATIC INVESTMENT PROGRAM ENROLLMENT

  1. Accomplish an Auto-Invest Form with BDO or Auto-Invest Form With BPI.

  2. Accomplish the following additional documents if you are an existing client who opened your account prior to the year 2014:
    1. Account Update Form (AUF)
    2. Provide a copy of one (1) Valid ID with 3x signature on the side.

  3. Submit the fully accomplished documents to our office.

  2. Bank documents must be submitted as original signed forms, scanned bank documents are strictly prohibited.

  3. Digitized signature are strictly prohibited.


ACCOUNT UPDATE

  1. Accomplish the forms
  2. For Individual or Joint Accounts:

    1. Fill out an Account Update Form
    2. Make a copy of a valid government issued ID
    3. Proof of latest billing statement that matches your present/permanent address on the Account Update Form

    Additional Requirements for In-Trust-For (ITF) Accounts

    1. In Trust For Account Form
    2. PSA Birth Certificate of beneficiary

    For Corporate Accounts:

    1. Fill out an Account Update Form
    2. Make a copy of a valid government issued ID of each signatory
    3. Make a copy of latest General Information Sheet (GIS)
  3. Submit fully accomplished Account Update Form together with photocopy of supporting documents through the following:
    1. If enrolled in our Electronic Mail Instructions – email scanned copy to ask@philequity.net
    2. If not enrolled in our Electronic Mail Instructions - submit original documents to our office.

REQUEST FOR CERTIFICATION

  1. Accomplish Certification Request Form
  2. Pay through the following mode of investment:

    1. Check should be payable to Philequity Management Inc.
    2. Deposit directly to any of our accredited bank accounts:

  3. Submit fully accomplished Certification Request Form together with your proof of payment through the following:

    2. If not enrolled in our Electronic Mail Instructions - submit original documents to our office.

Important:
  1. Cutoff time is 12:00NN. Certification Request Form received before 12:00NN will be processed same day and will be release on T+2 banking days. Otherwise, the request will be processed the next banking day.
